THE EFFECT OF ONLAY ISLAND FLAP URETHROLASTY FOR
HYPOSPADIAS WITH MILD OR WITHOUT CHORDEE

Objective: To
evaluate onlay island flap procedure in the repair of penile hypospadias
with of without chordee.
Methods: 25
patients ages ranged from10 months-3 years old ( averaged 1.8 years old )
were treated by onlay island flap urethrolasty. Procedures: 1. Parallel
incision(8mm apart) is made from meatus to the tip of glans to preserve
urethral plate; 2. To do circumcision incision around the corona; 3. To
deglove penile skin up to the base of penis; 4. Plication of tunica
albuginea should be done if penile chordee can not be released; 5. To make
island flap from the dorsal prepude; 6. To rotate island flap to the
ventrum and suture to the urethral plate to form neourethra. We prefer to
use 6/0 Dexon to repair urethra and put 8F silicone tube in the neourethra.
Results: 22
cases cured (88%), and 3 cases left a fistula postoperatively. One case
healed spontaneously and the other two were successfully repaired a year
later. All patients have normal appearance of penis and normal urination.
Conclusion: The
onlay flap urethroplasty is the effective method to repair hypospadias with
no chordee or with mild chordee.
