Objective: To study the incidence and iatrogenic
factors of neonatal triple acid-base disorders (TABD).
Methods: Arterial blood gas and serum Na+,
K+ and Cl- of 105 patients were determined on
admission and 24 hours later. Anion gap (AG) and potential HCO3-
were calculated.
Result: The total morbidity of TABD was 15.23%.
The morbidity of TABD in patients with moderate to sever HIE was
significantly higher than those with mild HIE (p=0.034). The difference
between the incidence rate of TABD in patients with hypoxemia and that in
patients without hypoxemia was also significant. The incidence of TABD was
high in patients treated with mechanical ventilation, NaHCO3 and
dehydrant than those without TABD.
Conclusion: To obtain AG and potential HCO3-
is very important to determine the existence of TABD. The correct use
of dehydrant
alkaline drugs and mechanical ventilation can reduce iatrogenic harmful effects of TABD on neonatal
patients.
