EFFECTS OF SMALL
DOSES OF CORTROSYN DEPOT COMBINATION WITH VITAMIN B6 AND LAMOTRIGINE
FOR THE TREATMENT OF INFANTILE SPASMS

Objective: To study the effects and safety of
small doses of Cortro-syn Depot combination with Vitamin B6 and Lamotrigine
for the treatment of infantile spasms.
Methods: Combination therapy consisting of
low-dose synthetic ACTH-Cortrosyn Depot [0.01��0.015 mg/(kg•d)] and high-dose Vitamin B6
[20-50 mg/(kg•d)] and
Lamotrigine was prescribed in 25 children with infantile spasms.
Results: As of 1 month after discontinuing the
Cortrosyn treatment, 22 of the 25 (88.0%) patients had experienced no seizures.
The mean interval until seizure control was achieved was 3.7��2.0 days (range 1.5��6.0 days) after the start of treatment
with Cortrosyn. 25 patients have been monitored for a mean of 20.2��14.1 months (range 6��30 months), 2 patients (9.1%) have had
recurrences of infantile spasms, and no patients had other types of
seizures. 9 of the 25 patients (36.0%) have had transient symptoms of
increased irritability and excitability, 7 of the 25 patients (28.0%)
manifested hyperorexia, but none of the patients develop more serious side
effects.
Conclusion: Combination therapy with low-dose
Cortrosyn Depot and high-dose Vitamin B6 and Lamotrigine appears
to be a more effective and safer therapy with very few side effects and
lower recurrence for the treatment of infantile spasms.
