Shanghai Children��s Medical Center, Shanghai, China Objective: Children with congenital
heart disease frequently have an enlarged heart, left-to-right shunt,
pulmonary hypertension, and recurrent respiratory tract infection before
surgery. During post surgery stage, lung atelectasis, impaired cardiac
function, and depending on ventilator are also common that slows down the recovery. Therefore, the
purpose of this paper is to describe the process and implication of
establishing chest physical therapy (CPT) for cardiac surgery children. Methods: One designated
nurse acquired knowledge and skills of CPT from a 3-month training in Israel Schneider Children��s Medical Center. Three clinical indexes, for example: oxygen saturation level,
arterial PO2, and arterial PCO2 level were collected from 36 children in cardiovascular intensive unit (CICU). All children serve as their own control. All clinical
indexes were collected 30 minutes after regular laryngeal suction and 30
minutes after completion of the CPT.
Results:
Clinical implementation
CPT has received a positive outcome from parents and physicians. CPT is
implemented in Shanghai Children��s Medical Center for one year; a total of
347 children received CPT,
and has generated $33630 RMB revenue from this practice.
Ninety-five nurses received CPT training and 142 parents got CPT
health teaching. All indexes collected in CICU were analyzed by T-test. The mean differences of SO2,
PCO2, and PO2 level before and after CPT were all statistical significant (P<
0.001). Conclusions:
CPT practice is
important and it needs administrative support to implement the work. The clinical outcome of CPT is encouraging
and parents also have expressed positive feedback. The clinical indexes of SO2,
PCO2, and PO2 after receiving CPT were better than not receiving CPT. This
preliminary data indicate that CPT could decrease the complication
associated with cardiovascular surgery and could promote the post-operative
recovery.
